4-H Enrollment

Member Forms 

4-H Enrollees must complete enrollment, code of conduct, and medical release forms. The enrollment fee for the 2026-2027 4-H year is $30.00, not to exceed $90.00 per household.

4-H members will enroll through 4HOnline at https://4honline.com/.  Enrollment opens October 1st and closes November 1st for returning members.  New members have a grace period for enrollment.

Enrollment assistance can be found at https://cornell.box.com/v/CHQ4-HEnrollment, by making an appointment at the office, or coming to the enrollment help night 4:30-6pm October 8th at the JCC Carnahan Center in Jamestown, NY.

Volunteer Enrollment
