4-H vouchers are very important because they are the only way the Fairboard knows what you'll be bringing to the fair. A legible, complete voucher is very helpful to club leaders, animal project leaders, as well as the 4-H entry clerk so that they can accurately plan for fair week. Thank you for taking the time to complete your 4-H voucher accurately and submitting it on time.

How to complete your fair voucher:
12. Exhibitor’s Signature: 4-H Member must sign and date fair voucher.
13. Parent’s Signature: All vouchers must include the date and parent’s signature.
14. Leader's Signature: Club leaders or Animal Project Committee member's signature verify that is correct and complete.
15. Number of animals: Each animal receiving a premium must be counted.
16. Stall Requests: Only applicable to Horse Exhibitors.
17. Please make note of the photo release checkbox on the voucher directly below the entry lines. If you give permission for CCE Chautauqua to use photographs of your child in future publications please check the box.
18. Parental Permission Form: Must accompany fair voucher if you have an animal entry or are a Junior Superintendent.
Please describe items for Department 12 (4-H Building) in detail as well as state the level of ability (i.e. beginner, intermediate, advanced) when necessary. It is hard to determine if the item is large or small or what level the 4-H’er is in the project when all the description says is “craft”.
Data entry is much more efficient if vouchers are filled out by department, keeping each department and section entry together and in order (i.e. keep all Section 12 entries together).
Members are not paid a premium for showmanship classes, but should still list them on the voucher for date tracking purposes and for the creation of animal show books.
It is recommended that a parent fill out the voucher for members that are under the age of 12. That way, it will be legible.
It will be the Organizational Leader’s responsibility to double check the vouchers before they are turned in. The voucher must be signed by the leader, 4-H’er and parent..
Remember that each youth may exhibit ONE item that was not listed on his or her voucher. Please see the Entry Clerk at the kiosk for more information.
The 4-H Office does not provide tables for club booths. When Evaluation Day is over, the tables used are taken down. It is the club’s responsibility to provide tables for their booths.
Vouchers MUST be turned in by Friday June 14th!
Club leaders; please fill out a voucher for your booth at the County Fair.
Attention Cloverbuds: If you are showing an animal, fill out a voucher and the sign parental permission form. You will receive three entry tags for 4-H Building exhibits from your club leader. Do not put 4-H Building exhibits on your voucher.
